Ipswich appeared poised to cause an upset when George Hirst neatly tucked away the opener shortly after the break. However, Ryan Yates equalised for Forest around the 70-minute mark, restoring parity.
Both teams were unable to find a decisive goal during regular time or in the extra period, leading to a penalty shootout at the City Ground.
The first four penalties from both sides were expertly executed, but after Callum Hudson-Odoi successfully converted Forest’s fifth shot, Jack Taylor saw his penalty saved by a diving Matz Sels, triggering ecstatic celebrations among the home fans.
Next up for Forest is a clash against Brighton and Hove Albion at the Amex Stadium, following Brighton’s victory over Newcastle on Sunday.
